Additional Information
The muscle biopsy should be selected from a moderately involved muscle on the basis of electromyography or other means of clinical evaluation. The muscle selected for biopsy should be on the contralateral side since electromyography produces histological changes which may mimic muscle disease. A detailed history is essential including current or recent use of steroids, familial history, clinical course, distribution and severity of the disease, laboratory findings, and results of electrophysiological studies. This form is available by calling the Pathology Office at 419-291-4370.
