Collection Instructions

For Inpatients – At the patient’s bedside, after positive identification is made, the patient’s blood sample is collected. The tube is labeled with the peel-off label from the band, bearing the patient’s name, hospital number, current date and collector’s initials (or tech code). The Blood Bank requisition form must be completed, and dated and signed by the phlebotomist (or tech code).

Rejection Criteria

Gross hemolysis, drawn in serum separator tube, requisition not signed by phlebotomist, specimen not properly labelled.
